Step 2: Schedule Your Consultation

Once your questionnaire has been reviewed, you will receive instructions for scheduling your appointment.

Step 3: Create Your Personalized Plan

During your visit, we will review your health history, symptoms, goals, previous testing, and concerns in detail. Together, we will create a personalized plan to help you improve your health today while also protecting your future health.

Pricing & Insurance

Cash-pay pricing for hormone, metabolic, and menopause care, serving

patients throughout Colorado.

I do not accept insurance and am a cash-pay practice. Lab testing and

prescriptions may be submitted to your insurance when applicable, and

a superbill is available upon request.

Patients are expected to maintain a relationship with a primary care

physician. This practice complements, but does not replace, primary care.

Options:

1. Consult-Based Care

For flexible, as-needed scheduling:

• New Patient Consultation: $400

• Follow-Up: $200

• Extended Follow-Up: $275

2. The Alignment Method: $300/month:

For patients who want ongoing continuity rather than isolated visits:

one monthly touchpoint, secure messaging (1 to 2 business day

response), lab review, and priority scheduling. New or complex

concerns are addressed through a scheduled visit rather than

messaging, billed at your member follow-up rate. No urgent, same-day,

or after-hours care. Billed monthly, with 10% savings for a 6-month

commitment.

3. Additional Services

- Forms, letters, and documentation: starting at $50

- Standalone lab or chart review: starting at $75

- Prior authorization support: starting at $50

Policies

This practice does not offer urgent, same-day, or after-hours care.

For emergencies, call 911 or go to your nearest emergency room.

Payment is collected at booking (or at the start of each billing

cycle for membership). 48 hours' notice is required for cancellations

or rescheduling; late cancellations and no-shows are billed at the

full visit fee. Membership is month-to-month and cancellable with 30

days' notice; unused services don't roll over or carry cash value.
