• No Doctor-Patient Relationship Established: Browsing this website, submitting an inquiry via a contact form, or scheduling a preliminary introductory call does not establish a formal physician-patient relationship with Dr. Alexis Michopoulos. A doctor-patient relationship is only established once you have completed our formal intake onboarding paperwork, consented to treatment, and attended your initial clinical consultation.

• Emergency Medical Care: This practice does not provide emergency, urgent, or same-day medical care. If you are experiencing a medical emergency, life-threatening symptoms, or severe psychiatric distress, call 911 immediately or go to the nearest emergency room.

• Complementary Care Requirement: Dr. Alexis Michopoulos provides specialized, non-urgent consultative care focusing on perimenopause, menopause, hormonal health, and metabolic longevity. This practice complements, but does not replace, comprehensive primary care. All patients are required to maintain an active relationship with an independent primary care provider for routine health maintenance, preventative screenings, and acute illnesses.

2. Telehealth Services & Geographic Limitations

• Colorado Licensure: Telehealth consultations and clinical services are offered exclusively to individuals physically located within the State of Colorado at the time of their appointment, in compliance with state medical licensing regulations.

• Telehealth Technology: Consultations are conducted via secure, HIPAA-compliant audio-video technology. While telehealth provides significant convenience, you acknowledge that remote care has inherent limitations compared to an in-person physical examination.

3. Payment, Insurance & Practice Policies

• Direct-Pay / Cash-Pay Model: Dr. Alexis Michopoulos operates as a direct-pay, cash-pay practice and is not an in-network provider with any commercial health insurance plans, Medicare, or Medicaid.

• Payment Terms: Payment for all consultations is collected in full at the time of booking. For patients enrolled in The Alignment Method, monthly membership fees are billed automatically at the start of each billing cycle.

• HSA/FSA & Superbills: Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA) may be used for qualifying clinical services. Upon request, we will provide an itemized receipt (superbill) containing diagnostic and procedure codes that you may submit to your health plan for potential out-of-network reimbursement. We make no guarantees regarding whether your insurer will reimburse any portion of our fees.

• Cancellation & Rescheduling: We require at least 48 hours’ notice to cancel or reschedule a confirmed consultation. Appointments canceled with less than 48 hours’ notice, as well as missed appointments (no-shows), are billed at the full visit fee.

• Membership Terms: The Alignment Method continuity care is billed on a month-to-month basis and can be canceled at any time with 30 days’ written notice. Unused monthly touchpoints or services do not roll over or hold cash value.

4. Notice of Right to a Good Faith Estimate (No Surprises Act)

Under Section 2799B-6 of the Public Health Service Act and the federal No Surprises Act, health care providers are required to provide self-pay (uninsured or out-of-network) individuals with an estimate of the bill for medical items and services before those services are provided.

• You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare service.

• In our practice, pricing transparency is standard: all fees for initial evaluations, follow-up consultations, and ongoing continuity memberships are clearly posted on our website prior to booking.

• You may request a formal written Good Faith Estimate at any time before scheduling a service. If you schedule a service at least 3 business days in advance, a written estimate will be provided within 1 business day of scheduling; if scheduled at least 10 business days in advance, it will be provided within 3 business days.

• If you receive a bill from this practice that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill through the federal dispute resolution process.

• HIPAA Compliance: We maintain strict physical, electronic, and administrative safeguards to protect your Protected Health Information (PHI) in full accordance with the Health Insurance Portability and Accountability Act (HIPAA). Clinical notes, lab results, and personal health data are stored exclusively inside our secure, HIPAA-compliant electronic health record (EHR) system.

• Website Communications: Standard website contact forms and unencrypted emails are intended for general scheduling inquiries only and are not secure channels for transmitting confidential health information. Please do not submit sensitive clinical details via general website forms.
